Intermediate Care and Reablement provide a service for people in their own homes, to maintain health and independent living as far as possible.

Home First is a fast paced and varied city wide service that provides support to Bristol residents upon leaving hospital. It is a 7-10 service to assess and support for any ongoing needs.

You will need to have NVQ/QCF level 2 in Care or be willing to undertake this, or have equivalent demonstrable experience. Effective communication skills are key as this role largely involves face to face work with vulnerable adults as well as communicating with allied health professionals and other agencies. You’ll need to work effectively as part of a team, and to be able to work without direct supervision and be self-reliant.

We have the following employment opportunities within the Home First team:

3 x Full time (37 hr) permanent posts

1 x Full time (37hr) fixed Term post until 31st December 2019.

Full time posts are on a split shift basis working 8-2 and 4.30-9pm over a 2 week rota.

  • Week 1 : working Monday, Tuesday, Friday, Saturday and Sunday
  • Week 2 : working Wednesday and Thursday.
  • Salary is £ 19945-£21166 + shift allowance.
  • Mileage is paid at 0.45 per mile.

2 x Part time posts – 21 hours per week (8-2 shift)

  • Working 8-2 on a 2 week rota
  • Week 1 : working Monday, Tuesday, Friday, Saturday and Sunday
  • Week 2 : working Wednesday and Thursday
  • Salary is £19945 - £21166 – pro rata
  • Mileage is paid at 0.45 per mile.

Please note this role is subject to an Enhanced DBS check.

A current driving licence and provision of a motorised vehicle is essential for this post, therefore business insurance will be required.
